What is Hospice Care?
Hospice care improves comfort and quality of life for patients with a terminal diagnosis. Hospice care is a benefit covered by Medicare/Medicaid and most private insurance. It is provided wherever patients call home.
The Hospice Care We Provide Includes:
- A Personalized care plan to meet each patient’s individual needs and wishes.
- Physician, Nursing and Aide care wherever the patient calls home.
- Pain and symptom management for improved quality of life.
- Coverage for medication, supplies and equipment related to the terminal diagnosis. Expert specialized care provided by trained staff through our specialty care programs.
- Spiritual, social and emotional support for both patients and their loved ones.
- Advanced Care Planning and additional resources.
- Vigil and bereavement support.
